Please list all instruments that you play in order of relevance, not necessarily proficiency (e.g., if you plan on playing violin with us, but are also a phenomenal guitarist, please list violin first). For each instrument, rate your skill using the following scale:

1 = Beginner
2 = Fair
3 = Intermediate
4 = Advanced

Decimals are encouraged if you feel you fall between levels. Please be honest with yourself: over-rating will lead to frustration and under-rating will lead to boredom. If you are out of practice, rate yourself at your current rather than previous level.

If you are a professional musician, please indicate your primary area using the following codes:

5 = Performer
6 = Educator
7 = Recent Graduate / Semi-Pro

The information provided on this page will be used to populate your profile on the Chamber Musician Directory.  All current members of CM-MDC and DCCM will be able to access this data.  Please see the "Chamber Programs Data Sharing" page on the DCCM Homepage for more information.

Except for "DCCM Program Interests" and "Music Education," your responses will only be used for internal program development purposes.
